If the patient is likely to meet the referral criteria, give them a Family History Form to complete at home and ask them to return the completed form to WMFACS, Clinical Genetics Unit, Birmingham Women’s Hospital NHS Foundation Trust, Edgbaston, Birmingham B15 2TG

Within the Family History Form, please complete section C on page 1 with your own details before giving it to the patient.
