Apply to become a Volunteer

If you feel you can spare a few hours a week, to do something rewarding and worthwhile, helping us to enhance a health care service that goes that extra mile, then please complete the online application form below.

You will also have to complete a Health Screening Form, Ethnic Monitoring Questionnaire a Criminal Records Bureau Disclosure form and provide details of two references.   In addition, please bring with you three pieces of evidence of your identity. These could be:

  • A birth certificate and/or a passport and/or
  • A driving licence
  • A bank statement or credit card statement with your name and address on
  • A utility bill
  • A letter from school/college/Government Agency, i.e. Job centre
  • An NHS Doctor’s card with name and address

After your application form is received, you will be invited in for an informal interview where the above forms and CRB can be completed and your evidence copied.
